Registered Nurses (RNs) and licensed Vocational Nurses (LVNs) work with your doctor(s) to provide home care services. Their assignment, establishment of nursing care plan, skilled treatment, and support provide the highest quality of care you need to recover at home. A registered nurse is available for admissions, urgent situations, or questions 24 hours a day.
Home Health Aides are Certified Nursing Assistants who provide aid in personal care and daily living as directed by and under the supervision of the RN.
- Assessment of patient's rehabilitation needs
- Range of motion exercises
- Gait training
- Instruction in using a walker and cane
- Pain management
- Education on safety precautions
- Ultrasound/Heat treatment
- Teach techniques to improve and restore the level of independence in ADL
- Design, Fabricate, and fit orthotic and self-help devices
- Communication rehabilitation (Speech Noise/Language)
- Swallowing Evaluation and Rehabilitation
- Assessment of emotional and social factors related to the patient and family
- Evaluate the home and family environment
- Counseling regarding financial needs, emotional support and adjustment to illness
- Assistance in financial matters
- Coordination of community resources
Provided as ordered by the physician in coordination with independent DME suppliers.
